Acute Transitional Care Nurse New

Are you a Registered Nurse passionate about patient-centered care, smooth care transitions, and making a real impact across the healthcare continuum? Join Pride Health with our leading geriatric primary care client within their Comprehensive Clinical Management team.

In this telecommute role, you’ll leverage your clinical expertise to support patients as they transition between care settings—hospital, home, skilled nursing, or ambulatory. You’ll work closely with primary care providers and interdisciplinary care teams to coordinate care plans, ensure continuity, and drive improved outcomes, without the stress of shift work or weekends.

*Applicants must reside within Treasure Coast, Tampa, Citrus, Northeast Florida, or the Orlando area*

Shift: M-F 8a-5p -Remote

Pay: $40-$46 Hourly

Contract Length: 6 months

Duties:

• Support patients and families across settings, from hospitals to homes
• Coordinate discharge planning, home care, follow-up, and social services
• Conduct outreach and support remotely or in-person (as needed)
• Guide patients using motivational interviewing and coaching strategies
• Advocate for continuity of care and optimal resource utilization
• May be required to visit patients in their homes within the coverage area

Requirements:

• FL RN license or compact
• Bachelor's in Nursing (or Associate's with 4 years of clinical experience)
• 2 years of clinical experience in hospitals, clinics, or physician offices
• Experience with care transitions, utilization review, or discharge planning preferred
• CCM cert or HMO experience preferred
